How to Create a New Notion Database Page Using a Page Template?


This guide shows you how to use a Notion page template when creating new pages from a form submission. Perfect if you want every new entry in your Notion database to follow the same layout automatically.


What You’ll Need


  • A Notion database
  • A Notion page template created inside that database
  • An existing form connected to your Notion database


Step 1: Create a Page Template in Notion


  1. First, you need to create a template inside your Notion database.
  2. Open your Notion database
  3. Click the ▼ next to “New”
  4. Select + New template
  5. Design your page layout (headings, properties, content, etc.)
  6. Save the template

Step 2: Open the Form Editor


  1. Go to your Form Edit Page
  2. Make sure the form is already connected to the correct Notion database


Step 3: Open Form Settings


  1. Click Settings (upper-left corner of the form editor)
  2. Navigate to Submission Settings


Step 4: Select Your Notion Page Template


  1. In Submission Settings, find the Notion Template option
  2. Select your preferred Notion page template from the list
  3. Click Publish Form or Save Changes





✅ All new submissions will now create new Notion database pages using the selected template

Every time someone submits your form, Notion will:


  • Create a new page in your database
  • Apply the selected page template automatically
  • Keep your entries clean, consistent, and organized
