How to Create a New Notion Database Page Using a Page Template?
This guide shows you how to use a Notion page template when creating new pages from a form submission. Perfect if you want every new entry in your Notion database to follow the same layout automatically.
What You’ll Need
- A Notion database
- A Notion page template created inside that database
- An existing form connected to your Notion database
Step 1: Create a Page Template in Notion
- First, you need to create a template inside your Notion database.
- Open your Notion database
- Click the ▼ next to “New”
- Select + New template
- Design your page layout (headings, properties, content, etc.)
- Save the template

Step 2: Open the Form Editor
- Go to your Form Edit Page
- Make sure the form is already connected to the correct Notion database
Step 3: Open Form Settings
- Click Settings (upper-left corner of the form editor)
- Navigate to Submission Settings
Step 4: Select Your Notion Page Template
- In Submission Settings, find the Notion Template option
- Select your preferred Notion page template from the list
- Click Publish Form or Save Changes

✅ All new submissions will now create new Notion database pages using the selected template
